The Palo Verde Healthcare District (PVHD) has announced a significant financial development that, upon final government agency approval, will allow Palo Verde Hospital to remain open and continue serving the Palo Verde Valley/Blythe community with a balanced operating budget through the end of the fiscal year (June 30, 2026).
The District has secured a $4 million loan to participate in California’s Medi-Cal Intergovernmental Transfer (IGT) program, administered by the California Department of Health Care Services (DHCS). Through this program, Palo Verde Healthcare District submitted an IGT pay-in of $3,445,595. Based on established program methodology, the District expects to receive approximately $9.9 million within about 60 days, subject to DHCS confirmation that the contribution qualifies as permissible local funds.
The loan will be repaid entirely with local funds, consistent with DHCS requirements prohibiting the use of federal funds for IGT repayment.
“This is welcome and very encouraging news for our patients, our employees, and the entire Palo Verde Valley,” said Carmela Garnica, Chairperson of the Palo Verde Healthcare District Board of Directors. “This funding creates a real path forward to stabilize hospital operations and protect access to local health care while the District continues to develop its long-range financial and hospital restructuring plan with the aid of the Chapter 9 bankruptcy process.

It’s an important issue that did not go unnoticed by local government officials, especially Riverside County District 4 Supervisor V. Manuel Perez, Riverside County Executive Officer Jeff Van Wagenen, staff at Riverside University Health System and the board and staff of Desert Healthcare District – all of whom have been working hard to seek a sustainable solution for the major financial challenges that impact Palo Verde Hospital and rural hospitals throughout California and the U.S.
What the IGT Means for Palo Verde Hospital
The IGT is a long-standing government financing mechanism that allows public health care entities to contribute local funds toward the state’s required Medi-Cal share, thereby enabling the state to access federal matching funds for Medi-Cal services. These combined funds are returned to participating providers through managed care plans, helping to address chronic under-reimbursement and timing gaps that disproportionately affect rural and safety-net hospitals.
For PVHD, participation in the IGT program provides near-term liquidity that will:
- Keep Palo Verde Hospital open and operating while funds are reconciled and disbursed;
- Nearly 80 employees in the City and region will continue working to provide critical medical services to the community;
- Support a court-supervised financial workout plan in coordination with the bankruptcy court, aimed at restoring long-term stability – not exiting District ownership or local control; and
- Strengthen cash flow as the District continues broader restructuring efforts already underway.
While final disbursement remains contingent on DHCS approval, PVHD has completed all required steps to participate and is actively working with state officials to finalize the process.

Context
PVHD initiated an emergency reorganization in 2025 in response to severe financial pressures common to rural hospitals and later authorized a Chapter 9 filing to protect essential services while restructuring obligations. The anticipated IGT proceeds are a critical component of the District’s strategy to develop a sustainable workout plan with the bankruptcy court to stabilize operations, support employees, and ensure continued access to emergency and outpatient care in the Palo Verde Valley.
